By registering for and participating in the Sky High Youth Services Basketball Tryouts, I acknowledge and agree to the following: I understand that participation in basketball activities involves physical exertion and inherent risks, including but not limited to injury, illness, or accidents. I voluntarily assume all risks associated with participation. On behalf of myself and/or my child, I hereby release, waive, and discharge Sky High Youth Services, its staff, coaches, volunteers, and the host facility (St. Barnabas Church) from any and all liability for injuries, damages, or losses that may occur during or as a result of participation in tryouts. I certify that my child is physically able to participate and has no medical conditions that would prevent safe involvement. In the event of an emergency, I authorize staff to seek medical treatment if necessary. I understand that: A parent/guardian must agree to this waiver for all participants under 18 No participant will be allowed to try out without a completed waiver Participation is voluntary and can be withdrawn at any time By completing registration, I confirm that I have read, understood, and agree to this waiver.
